Transferring to Mindbloom From Another Provider

Moving your care from your existing provider to Mindbloom

Can I transfer to Mindbloom if Iโ€™ve been with another ketamine provider?

Yes. You don't need permission from your current provider to start with Mindbloom. Your Mindbloom clinician will review your treatment history and determine your eligibility during your initial consultation.


Do I need to notify my current provider?

It's not required, but we recommend giving your current or previous provider a heads-up that you're making the transition. It helps keep your care coordinated and your records up to date.


Do I need a discharge letter?

Only in certain cases. A discharge letter is required if:

  • You have an active ketamine prescription with your current provider, or

  • You've received IV or in-clinic ketamine treatment within the last 30 days

If neither of these applies to you, no discharge letter is needed.

Your previous provider simply needs to confirm in writing that they will no longer be prescribing ketamine for you. The letter should include:

  • Provider's name, clinic name, and contact information

  • Dates and total number of ketamine treatments received

  • Date of last treatment or prescription cancellation

  • Dosages and route of administration used


Do I need to wait for my discharge letter before getting started with Mindbloom?

Not at all - we encourage you to get started as soon as you're ready. You can reach out to your previous provider for the letter while you prepare for your clinician consult. If you haven't received it by the time of your appointment, you can still meet with your clinician; they may simply require it before moving forward with your prescription.


How do I submit my discharge letter?

After you purchase your program with Mindbloom, you can securely email it to support@mindbloom.com or upload it directly through chat in your Mindbloom mobile app. Our team will add it to your chart so your Mindbloom clinician can review it before your consult.
