Accessing Medical Records or Health Information through Mindbloom
Mindbloom prioritizes your privacy and the secure handling of all health information. If you’d like to share your treatment information with another healthcare provider—or access your own clinical records—you can do so through the process outlined below.
For Clients Requesting Their Own Records
If you’d like a copy of your medical records or other health information:
Message Mindbloom Support to start the request process.
Support will provide next steps and any required forms for release.
For Third Parties (e.g., Other Healthcare Providers)
If another provider or representative is requesting health information, they’ll need to submit:
A completed Release of Information (ROI) form
A copy of your government-issued ID to verify authorization
To ensure accurate processing:
Documents must be submitted without e-signatures (wet or scanned signatures only).
We recommend CC’ing the client on the request email for transparency.
All documents should be sent to support@mindbloom.com.
For Third-Party Representatives:
If you are requesting health information on behalf of someone else, the following is required:
A completed Release of Information (ROI) form.
A copy of the client’s government-issued ID to verify authorization.
Ensure all documents align with Mindbloom's submission standards, including avoiding the use of e-signatures. For added accuracy in processing, third-party representatives are encouraged to include the client in communications by CC’ing them on emails when sending the required documents to support@mindbloom.com.
Submission and Processing
Email all required documentation to support@mindbloom.com.
Mindbloom may reach out if any additional information is needed to complete your request.
Requests are processed securely and in compliance with HIPAA and privacy standards.
Important Notes
E-signatures are not accepted on ROI forms or ID documents.
Requests for clinical notes, session summaries, or other official records must follow this same process.
Superbills (for insurance reimbursement) are available separately and do not require this process.
